Documents Required For Health Insurance

An Insurance policy is a legal contract, therefore, Insurance Documents are required to prove the legitimacy of the deal.

The Insurance Policy is issued once the Health Insurance Company verifies all the provided documentation. At the time of the Insurance Claim, a set of documents need to be submitted for Claim Settlement.

Documents Required for Buying Health Insurance

  1. Age Proof

    It is one of the most important documents in Health Insurance. You need to submit the Age Proof of all the members covered under the Health Insurance Policy. Below are some of the acceptable documents that you can submit as Age Proof:

    • Aadhaar Card
    • Birth Certificate
    • Passport
    • Driving License
    • Pan Card
    • Voter’s ID
    • 10th or 12th Marksheets
  2. Address Proof

    You also need to submit the permanent Address Proof. Following are the documents that can be submitted for the same:

    • Aadhaar Card
    • Telephone Bill
    • Electricity Bill
    • Passport
    • Ration Card
    • Driving License
    • Voter’s ID
  3. Identity Proof

    You can provide the following documents as Identity Proof:

    • Voter ID
    • Aadhaar Card
    • Passport
    • PAN Card
    • Driving License
  4. Income Proof

    Income Proof allows the policyholder to decide on the Sum Assured & the total cover that can be given to the Insured.

  5. Photographs

    Passport Size Photographs are required by the Insurance Company.

  6. Medical Report

    You have to undergo a Medical Examination and also have to submit the test report of the Medical Exam. This Is usually applicable when an Insurance Company asks the results for a Pre-Medical Check-Up

Documents Required for Claim Settlement under Health Insurance

Cashless Claim Process- When you’re admitted to a Network Hospital

  • Request Form to be submitted with 24 hours of Emergency Hospitalisation
  • Request Form to be submitted at least 4-5 days before hospitalization (in the case of a planned hospitalization)
  • Duly Filled Claim Pre- Authorization Form
  • Valid ID Proof of the policyholder
  • Documents- Aadhar Card, Pan Card, Voter ID, Driving License
  • Insurance Card
  • Policy Document Copy
  • Duly Filled KYC Form (if required)
  • Any other document, if required by the Insurance Company

Reimbursement Claim- When you’re admitted to a Non- Network Hospital

  • Duly signed & filled Claim Form
  • Valid ID Proof
  • Insurance Card
  • Policy Copy
  • Medical Certificate signed by the doctor
  • Doctor’s Medical Prescription indicating Diagnosis
  • Doctor’s medical prescription suggesting Medicines, Treatment & Diagnostic Tests
  • Pathological/ Diagnostic Reports
  • Original Medical Bills
  • Discharge Certificate/ Summary of the policyholder
  • Ambulance Receipts
  • FIR / MLC Copy (in case of an accidental claim)
  • Original Pharmacy Bills
  • NEFT Details to credit Claim Settlement
  • Any other document, if required by the Insurance Company
